Step 1:
After deciding to purchase the Medical Records 2 Go, LLC form you will need to click on the Purchase Now button in order to make your purchase.
After purchasing the form you will be directed to our download page. This is where you will be able to download our propriatary medical record form and other free forms, such as our: Emergency Items List, Adult Vaccination Record Form, Child Vaccination Record Form and Pet Health Record Form. (Free forms are for print use!)
If you do not have the latest version of Adobe Reader, click here to download the latest version of Adobe Reader you will then be able to enter, edit and/or change your data you have entered. Please be sure to save your changes.
Step 2:
Once you have opened the form, you may begin filling out the form. You can use your mouse or tab to move to the various fields for inputting your information. You can save the file and come back later to add or change any information you have entered. You can print the form and save it, or save the form on a flash drive for portability
