I had major surgery in 2010, and during the recovery time at home, I passed out and my wife called (911) for the paramedics to come to our home.

My wife handed them the emergency paramedic form with all the pertinent medical information about me. The paramedics took the information they needed and wrote it down on their forms.
We went to Thomas Jefferson University Hospital, taking the emergency paramedic form with us, they in-turn, entered the information into their computer system - which enabled the doctors to come in and immediately give medical treatment.
As a result of having the paramedic form filled out before hand, the situation went smooth, eased our anxiety and also aided the hospital emergency team at Thomas Jefferson University Hospital.
